Here's what our communication with referring physicians looks like:

At the Start of Care: You'll receive confirmation that we've received your referral, verification of insurance and eligibility, and a summary of the initial care plan. If we have questions about your orders, we'll reach out directly rather than letting them sit.

During Care: You'll receive regular clinical updates on your patient's progress, including any significant changes in condition, medication concerns, or functional status changes that require your attention. We communicate through your preferred channel, whether that's fax, secure message, phone, or your EHR system.

At Discharge: You'll receive a comprehensive discharge summary that outlines what was accomplished, where the patient stands functionally, and any recommendations for follow-up. No gaps. No guessing what happened between referral and discharge.

When Something Urgent Comes Up: You'll hear from us immediately. We don't wait for the next scheduled update to tell you about a change in condition, a fall, a new symptom, or anything else that requires your clinical judgment. Our team is trained to escalate quickly and communicate clearly.

Clinical decisions are made by clinicians, not administrators. That means your patients receive care plans grounded in evidence and professional judgment, not driven by corporate metrics.

We don't let referrals sit. Our goal is to initiate care within 24 to 48 hours of receiving a referral, and we keep you informed throughout the process. If there's a scheduling issue, a coverage question, or a clinical concern, you'll know about it right away.

901 Home Health Care provides a comprehensive range of skilled home health services, all delivered by licensed professionals and coordinated under a single care plan.

  • Skilled Nursing (SN): Medication management, wound care, chronic disease monitoring, post-surgical care, patient education, and care coordination.
  • Physical Therapy (PT): Strength and conditioning, balance and fall prevention, gait training, range of motion restoration, pain management, and home safety assessment.
  • Occupational Therapy (OT): Activities of daily living, home modification, upper body rehabilitation, adaptive equipment training, cognitive-perceptual skills, and energy conservation strategies.
  • Speech-Language Pathology (SLP): Speech and language rehabilitation, cognitive-communication therapy, dysphagia evaluation and treatment, voice therapy, and aphasia treatment.
  • Medical Social Work (MSW): Community resource connection, insurance navigation, care transition support, psychosocial counseling, caregiver support, and advance care planning.
  • Home Health Aides (HHA): Bathing and dressing assistance to prevent skin breakdown.

All disciplines communicate internally and coordinate with your office to ensure continuity and alignment with your treatment goals.

The Referral Process

Referring a Patient Takes Minutes. We Handle the Rest.
We've designed our referral process to be as fast and frictionless as possible. Here's how it works.

Step 1: Send the Referral
Fax or submit a referral. We need the patient's basic demographic information, diagnosis, relevant medical history, notes from the patient's most recent visit, and your orders for home health services. If you have questions about what to include, call us and we'll walk you through it.

Step 2: We Verify and Confirm
Once we receive the referral, we verify insurance coverage and confirm eligibility. We'll contact your office to confirm receipt and let you know if we need any additional information.

Step 3: We Contact the Patient
We reach out to the patient or family to schedule the first visit. Our goal is to initiate care within 24 to 48 hours of receiving the referral.

Step 4: We Start Care and Keep You Informed
From the first visit forward, we manage the care plan, coordinate across disciplines, and keep you updated on your patient's progress. You stay in the loop without having to chase us down.
